What are constraints exactly?

A compulsion is an inner urge to think or do certain things (compulsive thoughts) that often feels uncontrollable to the person concerned. Most people with a Obsessive-compulsive They are well aware that their behaviour is excessive, irrational or even pointless. Nevertheless, they feel an enormous inner pressure that forces them to carry out these thoughts or actions.

Obsessive-compulsive thoughts are often of burdensome, often taboo content – for example in relation to violence, sexuality, illness or guilt. Compulsive acts, in turn, are undertaken as an attempt to neutralize the associated fear or unrest.

Two main forms:

Compulsive thoughts: Intrusive, intrusive thoughts, images or impulses that are perceived as excruciating. They are not voluntary and are usually at odds with one's own moral beliefs.

Compulsory acts: Repeated behaviors or rituals performed to reduce anxiety or discomfort or prevent a feared misfortune.

In addition, there are mental constraints, inner rituals such as silently repeating certain words, praying or mentally playing through situations.

2. Where does the habit end, where does the compulsion begin?

At first glance, compulsions resemble many everyday habits. Everyone knows the need to check something, clean it up, or arrange things symmetrically. However, the difference lies in Suffering pressure and the Feeling of loss of control.

Compulsive acts are not simply ‘ticks’ or ‘masks’. Over time, they often take up more and more space in everyday life, claim hours per day and prevent a normal life. They are also rarely voluntary – the person concerned does not feel that he or she is acting of his or her own free will, but is forced to do so by an internal power.

Obsessive-compulsive thoughts, on the other hand, are not experienced as a wish, but as a threat – often associated with extreme shame and fear.

3. How do constraints arise?

The emergence of constraints is complex. It is a combination of biological, psychological and social factors. Research is talking about a so-called Bio-psycho-social model.

Biological causes:

Disorders in certain brain regions, especially in the so-called fronto-striatal control loop.

An imbalance in the neurotransmitter balance, especially in serotonin.

Genetic preload: Obsessive-compulsive disorders occur more frequently in families.

Psychological causes:

Early learning experiences, for example in an environment with excessive rigor, high moral control or fearful occupation.

Excessive sense of responsibility: Many people feel responsible for everything and everyone.

Low confidence in one's own perception: They doubt whether they did things correctly or remembered them.

Social factors:

Traumatic experiences, such as abuse, separation or neglect.

Critical life events or constant overload in everyday life.

Often, compulsions begin in childhood or adolescence and develop gradually over years.

4. Forms and examples

Constraints can occur in many ways. Some are visible, such as washing compulsions or control rituals, others run internally and go unnoticed by outsiders.

Washing and cleaning compulsion

This form is one of the most common constraints. The fear of dirt, germs or disease causes those affected to excessively clean or wash. This can lead to bloody skin injuries.

Obligation to check

Here, the fear of causing harm through negligence dominates: Did I turn off the stove? Is the door locked? Did I hurt someone? Affected persons check several times – often also with aids such as photos or video recordings.

Compulsory order and symmetry

Objects must be precisely aligned or symmetrically placed. Often this is accompanied by a feeling of inner unrest, which only subsides after the ‘right’ arrangement.

Compulsory counting

Numbers have to be thought or spoken in a certain order, certain actions are repeated exactly as often. ‘Unfortunate numbers’ are avoided.

Mental compulsions and brooding compulsion

Thoughts such as ‘If I don’t think this correctly, something bad happens’ lead to endless mental loops or rituals. The constant questioning of moral decisions also falls into this area.

5. Everyday life with constraints: How does that feel?

Everyday life with constraints is often characterized by shame, isolation and exhaustion. Many sufferers lead a double life – they appear inconspicuous to the outside, but inside they fight non-stop.

Typical is the so-called ‘avoidance behaviour’: Situations or places that could trigger the coercion are avoided. This can lead to social isolation. At the same time, constraints often take up so much time that professional life, relationships and leisure time are severely impaired.

The emotional burden is high: Feelings of guilt, anxiety, feeling ‘crazy’ – many people with compulsions also develop depression or anxiety.

6. What happens in the brain during obsessive-compulsive disorder?

Obsessive-compulsive disorders can also be explained on a neurobiological level. Various studies show that certain brain regions are overactive in those affected – in particular the so-called fronto-striatal control loop, which is responsible, among other things, for the processing of errors, the assessment of hazards and the control of impulses.

Especially the connection between the orbitofrontal cortex, the basal ganglia and the thalamus seems to be disturbed in obsessive-compulsive disorder. This loop works like a filter in healthy people: It helps to sort out unimportant thoughts and focus attention on relevant things. However, for people with constraints, this filter remains open – threatening thoughts are not hidden, but push themselves again and again.

Another important factor is the messenger substance Serotonin. Serotonin helps regulate stimuli in the brain. An imbalance in this system can lead to an over-perception of internal warning signals, even though there is objectively no danger.

These biological processes explain why constraints are not easy to ‘think away’. They are not a question of weakness of will, but are deeply rooted in the function of the brain.

7. Common comorbidities (comorbidities)

Obsessive-compulsive disorder rarely occurs in isolation. In many cases, sufferers suffer from other mental illnesses at the same time. These so-called comorbidities often reinforce each other, making diagnosis and treatment more complex.

Typical comorbidities:

depression: Many people with compulsions develop depressive symptoms – such as constant overwhelm, self-doubt and social withdrawal.

Generalized anxiety disorder: A persistent, diffuse fear leads to compulsions being carried out even more intensively in order to alleviate this fear in the short term.

Social phobia: The fear of evaluation or rejection can be exacerbated by coercion – for example, when rituals are noticeable or experienced as embarrassing.

Eating disorders: Especially with restrictive eating behavior, forced structures can develop in terms of calories, amounts or rituals.

ADHD: When occurring at the same time, there are often problems with impulse control and attention control, which can additionally increase compulsions.

Comorbidities should be taken into account in therapy, otherwise symptoms may shift or relapses may accumulate.

8. Forms of therapy – explained in more detail

The treatment of constraints is possible – and in many cases very successful. It is important that it starts early and is individually tailored to the person.

Cognitive Behavioral Therapy (CBT) with Exposure and Reaction Prevention (ERP)

This is considered the most effective form of therapy. The basic idea: Exposing oneself consciously to the stimuli that trigger fear – but without Perform the usual compulsory ritual. Thus, the brain learns that the feared catastrophe does not occur.

Exemplary exposure: A person who is forced to wash touches a door handle – and suppresses the urge to wash his hands immediately. At first, fear increases, but over time it decreases by itself.

The method is carried out gradually and under therapeutic accompaniment. It takes a lot of courage, but it is highly effective.

medications

Especially so-called SSRI (selective serotonin reuptake inhibitors) such as sertraline or fluoxetine are used. They help to dampen the overactive anxiety loop in the brain and facilitate access to behavioral therapy. It often takes several weeks for the effect to start.

Other therapeutic approaches:

Depth psychological procedures: Reappraisal of childhood experiences or internal conflicts.

ACT – Acceptance and Commitment Therapy: Dealing with stressful thoughts without avoidance, focus on personal values.

Group therapy & Self-help: Exchange of experience, normalization, mutual support.

An integrative approach that combines different methods is often the most effective.

9. Constraints in the Mirror of History

What is now diagnosed as obsessive-compulsive disease has often been misunderstood or tabooed in the past. In the Middle Ages, people with constraints were considered ‘possessed’ or ‘blasphemous’. Rituals such as constant prayer or cleansing were interpreted religiously.

It was only with Sigmund Freud and psychoanalysis that a first psychological explanation emerged at the end of the 19th century: Compulsions were seen as an expression of repressed urges, often of a sexual or aggressive nature. This theory is obsolete today, but it helped to de-tabuise the subject.

Modern neuroscience shows: Compulsiveness is neither a weakness of character nor a sin – it is a treatable mental illness. Social interaction has improved, but many people still struggle with stigmatisation to this day.

10. Self-reflection: Am I affected?

Not everyone who tests something twice is obsessed. But there are warning signs. If you can answer ‘yes’ to the following statements, it might be useful to seek help:

I feel compelled to think or do certain things even though they seem nonsensical to me.

I spend more than an hour a day with rituals or thoughts that burden me.

I avoid certain places, people or situations for fear of triggering my compulsions.

I feel like I'm losing control of my thoughts or actions.

I am ashamed of my behavior and don't talk to anyone about it.

If you know of several points: You are not alone – and you don’t have to endure it. There's help.

11. Tips for those affected

Dealing with constraints is often a long process – but there are ways that can help to regain more control over one’s life. Here are some tried and tested strategies:

Acquiring knowledge: The better you understand your condition, the less threatening it will be. Books, podcasts, or reputable websites can help identify patterns.

Breaking the avoidance: Even if it is difficult, try gradually to avoid fear-inducing situations. Every little success strengthens your confidence.

Allow feelings: Fear, guilt, disgust – they are part of an obsessive-compulsive disorder. If you can stand them without acting, they will lose their strength over time.

Keeping a diary: Write down when compulsions occur, how you feel, and what helped you. This promotes self-awareness and helps to recognize patterns.

Don't give up: Setbacks are normal. Change takes time and repetition. Be patient with yourself.

Most importantly: Get some support. You don't have to go this route alone.

12. Tips for relatives

Constraints do not only affect the sick person – the environment is also often heavily burdened. Relatives are torn between compassion and helplessness. Here are some helpful principles:

Understand what a coercion is: It is not a ‘in lieu’, but a real mental disorder.

Do not participate – but also do not confront: If you carry the rituals with you, you stabilize the compulsion. If you're aggressive about it, you're creating stress. The middle ground is: Support, but don't take over.

Encourage instead of push: Therapy is a sensitive issue. Sensitive conversations often help more than pressure.

Maintaining Your Own Boundaries: You can also say "no". Take good care of yourself – this is the only way you can help in the long run.

Informing together: There are special offers for relatives – e.g. self-help groups or counselling centres.

13. History and forecast

Obsessive-compulsive disorders are usually chronic if left untreated. They can become more severe over years and lead to massive restrictions in all areas of life. However: With the right therapy, a significant improvement is possible – often even a far-reaching absence of symptoms.

The earlier a treatment begins, the better the chances of success. Also, relapses are not a sign of failure, but a part of the change process. It is important to stay tuned and get support even after prolonged stability when it is needed.

An open approach to the disease – in the family, in the circle of friends, at work – can also relieve the burden. Those who no longer have to hide take a part of their power from coercion.

conclusion

Compulsions are more than harmless quirks. They are a serious but treatable mental illness. They are a constant burden for those affected – but there are ways to deal with it. The main message is: You're not alone. There is help, there is hope, and there is life beyond coercion.

Change is possible – step by step. With understanding, patience and professional support.

Related links

www.zwaenge.de – German Society of Obsessive-Compulsive Diseases e.V.

www.angstselbsthilfe.de – Information and group offers

www.psych-info.de – Psychotherapist search in Germany

Click here for the appropriate podcast sequence

Silent Silence 003 Obsessive-Compulsive Disorders – When Thoughts Become Torment

📚 Further literature on Amazon (Affiliate ‑Links)

https://lautfunk.uber.space","https://www.facebook.com/sascha.markmann","https://www.instagram.com/lautfunk/","https://www.linkedin.com/in/sascha-markmann-b46019137/","https://de.pinterest.com/Raumwelle/","https://x.com/raumwelle","https://www.youtube.com/user/fellmensch","https://www.tumblr.com/lautfunk
4.
